Membership Guidelines
- Must stay on 6-month re-care program
- 2 business days are needed to cancel an appointment without penalty
- No refunds or premiums will be issued at any time if the participant chooses not to utilize the program
- Cannot be used in conjunction with another dental plan
- Balance of services in full must be paid at the time of service
- Non-refundable $50.00 reinstatement fee if plan lapses
